Also, know that we are here to serve and assist you and your family during difficult times. If you, or a family member, are experiencing an illness, death, or have a counseling need, please complete the referral form below. After your submission, a Partner Care Team member will be in touch with you within 24 hours.

A Partner Care team member will assist you as needed. If requested, applicable funeral assistance will be provided as available. Calvary Revival Church will assist with and/or host funerals for:
- Partners
- Partner’s Spouses
- Children of Partners who are 21 & under
- Partner’s parents, in some special cases
- Extended family members of high-capacity leaders, in some special cases
